Behavior Sheets

Behavior sheets are a useful tool. We use them in our BOCES classroom everyday. They are an integral part of the program.

Please see below for an explanation of the behavior sheets that our students will be using this year:

Students are responsible for asking teachers to sign their behavior sheet at the END of each period.

• Teachers  initial in the space on the right after filling out

• The targeted behavior is individual to each student.  Please base the score on the student’s behavior in the room for that period

• Teachers are to  let us know how the student is doing for homework completion (rating chart is at the bottom of the page)

• Teachers are to place any comments/concerns in the comment section.  Students may also use this space to write in homework

• Teachers  rate student behavior for the period they are in the classroom (rating chart is at the bottom of the page)

These sheets go home daily, are signed by parents, and are returned to the classroom.  They are used to monitor: patterns of behaviors, homework completion, modification of targeted behaviors,  success in the mainstream setting, etc.
